Deprecate ancient "debugging" tools.

Browse Source 
* logIO() and the global $xmlrpc_logging in XML-RPC.
 * log_app() and the global $app_logging in APP.
 * debug_fwrite(), debug_fopen(), debug_fclose(), and $debug.
